Three months after the events of the "Other Town", Thomas Kerr is attacked and mysteriously vanishes after being taunted by a animal like humanoid figure. Elaine begins suffering visions of the Other Town dimension which leads her to spit blood. At school, Claire and Evan begin working together and learn that they are the only ones who remember the events that happened three months ago. Ben Holloway, revealed to have been resurrected, escapes the Other Town and reunites with Mara Holloway, who introduces him to her boyfriend Bob. After school, Noah Pike crosses paths with a portal and deduces it is to the other dimension, which he sees a figure, but thinks nothing of it. In the Other Town dimension, the entity is revealed to be the "Warewolf", with it explaining to a severely injured Kerr. Warewolf kills Kerr stating that he took advantage of the erasers that occured months prior, and vows to eliminate the kids and take control of the main dimension. Sheriff Rowan Hale meets up with Evan and informs him of Kerr's death, leading to Evan to suggest that it relates to the other dimension.

Warewolf orders its followers, referred to as “minions,” to locate Gary Whitlock, a former associate of Thomas Kerr. The minions appear outside Whitlock’s store, attack him, kill several bystanders, and abduct Whitlock into the Other Town, delivering him to Warewolf. At Rowan’s request, Noah and Evan enlist Lucas to help investigate Kerr’s death. In the Other Town, Warewolf restrains Whitlock in a cage, claiming it wanted him to understand what he had helped conceal, and reveals that it was once human. Elsewhere, Linda Parks is attacked by a shadow entity resembling one of the beings defeated months earlier and is killed before Ben intervenes; it is revealed that Ben has been secretly experiencing visions of the disappearances, the deaths, and Warewolf itself. Warewolf later kills Whitlock and identifies itself as Jackson Miller, a boy who vanished during the first incursion in 1970. Ben confides in Evan about his visions and warns him that a wolf-like figure is responsible for the recent attacks. As the episode ends, Warewolf opens a portal and crosses into the main dimension.

Northpoint begins being covered in blood mysteriously as Sheriff Hale demands that all residents seek shelter and keep their homes locked. Evan and Elaine alongside Lucas, Claire, and Ben learn that the entity, that Hale dubs "Wolfman", is in their dimension and is from the Other Town. Warewolf attacks an apartment and he causes the owner to see visions and reads his mind to find out where the police is, so that he can "cleanse the souls" of the corrupt. Evan meets Mady Porter and the two agree to go on a date, ending with a kiss. Elaine is attacked by a minion and is dragged to a deserted area, where the others follow but the entire group are attacked by Warewolf, who vows to kill them, but not "yet" as he "has plans for them". After beating them, Warewolf leaves. Sheriff Hale and all the officers arrive, where Hale deems the teens not the intended target. Warewolf watches from the distance and summons a portal to the Other Town where he returns to. In hospital, Lucas reveals that Ben has been hiding something.

Evan recovers in hospital and meets signal analyst Alex Singh, who confirms he is aware of the Other Town and reveals he originated from the year 2077, though he refuses to explain future outcomes. Evan urges Alex to remain in Northpoint and help stop what is happening, but Alex avoids committing beyond observation. As the Warewolf escalates its actions, it begins using the blood of its victims to power a device capable of opening a larger, more stable portal to the Other Town. Ethan Mellis reappears and reveals that his best friend, Miles Porter—Mady’s brother—has returned, prompting Mady to confide in Evan and express her fear of losing him and her growing attachment to his family. Alex later discloses he was born in 2050 and constructed a machine that allowed him to travel back in time. Sheriff Hale responds to reports of a wolf-like figure and a strange machine, bringing him into direct contact with the teens, where Lucas confronts Ben, who admits he has been experiencing visions connected to the disappearances and apologizes for hiding them. Alex uncovers records indicating that Jackson Miller, a boy who vanished in 1970, was taken into the Other Town, though his survival remains uncertain. Elsewhere, the Warewolf gathers its minions and demands their loyalty as it works to “perfect” the world it now occupies.

In 1966, Jackson Miller encounters a spiritual entity that foretells his role in triggering an incursion that will give rise to an alternate dimension later known as the Other Town. In 1970, after sharing a quiet moment with his girlfriend Holly, she is killed in a freak accident, sending Jackson into a spiral of grief and substance abuse. After overcoming his addiction, Jackson is violently attacked by a group of bullies who blame him for Holly’s death; he retaliates by killing them and flees toward a mysterious portal. Crossing through it, Jackson finds himself trapped in a distorted version of Northpoint where time does not function normally. There, a shadowy presence attacks him and reshapes him into a wolf-like entity, naming him the Warewolf. In the present, Alex, Lucas, Evan, Mady, and Miles investigate a barn and discover the machine connected to the Other Town, only for the Warewolf to return, attack them, and demand they leave him alone before escaping with the device. Elsewhere, Sheriff Hale confronts Elaine and demands to know how she managed to contain the Other Town, realizing the current threat is rooted in choices made decades earlier.

Residents experience intrusive memories and emotional collapse as denial is stripped away, while Alex, Mady, Ethan, and Miles investigate sites marked by gravity-defying blood and realize the violence is psychological rather than lethal. Elaine confronts the Warewolf within the residual space of the Other Town and learns it is exploiting the weakened containment, punishing unresolved guilt indiscriminately as it begins to fracture under the weight of accumulated memories. Elsewhere, Alex encounters the Warewolf directly and is forced to relive a moment of his own past silence, collapsing after the entity leaves him alive but broken. Elaine records a warning that the Warewolf is dismantling denial itself and admits she does not know how to stop something that believes it is acting justly. By dawn, the blood fades but the damage remains, and Alex warns the group that the Warewolf is not an ending but the beginning of consequences, as the entity stands alone in the Other Town, visibly destabilizing while another unseen presence begins to stir.
